US DOT # is required. You can apply online @ http://www.fmcsa.dot.gov/registration/do-i-need-usdot-number & click on Register Online & click on FMCSA Online Registration. Please note that credit card information is required to verify your identity BUT there is NO charge for registration.
You must register your Business Name with the MN Secretary of State UNLESS you are using your legal name and operating as a sole proprietor. Visit http://www.sos.state.mn.us & Click on Business, Non-Profit & UCC OR call 651-296-2803 (9:00 a.m. – 4:00 p.m.).
One Time Registration – SWIFT Vendor ID is required by MnDOT for Month End Truck Reports. The Federal Motor Carrier Safety Administration requires Biennial Updates of the MCS-150. All entities under its jurisdiction are required to update their information every two years OR when you have an equipment change i.e. new equipment purchase. You are required to provide this update every two years even if your company has not changed its information, has ceased interstate operations since the last update, or is no longer in business and you did not notify FMCSA. Failure to complete a Biennial Update will result in deactivation of your USDOT number and may result in civil penalties of up to $1,000 per day, not to exceed $10,000. Updating your information is free. A PIN # is required & can be obtained by visiting the website @ http://safer.fmcsa.dot.gov.
Upon receipt of the PIN visit the website @ http://www.fmcsa.dot.gov & click on update your registration & click on Update Online. You will be required to enter US DOT, EIN & Credit Card Information for your electronic signature when filing your update. (There will NOT be a charge to your credit card the information is used for verification purposes only)!
On the MCS-150 under reasons for filing check Biennial Update or Changes & update form as required. Verify THAT your COMPANY NAME, CONTACT, ADDRESS & ANNUAL MILEAGE information are correct (make changes as needed), COMPANY OPERATION select INTRASTATE NON-HAZMAT CARRIER & INTRASTATE HAZMAT (ONLY if you have a Hazmat endorsement on your license), OPERATION CLASSIFICATION should be A (authorized for hire) & any other options that apply, CARGO CLASSIFICATIONS should be CONSTRUCTION & OTHER/AGGREGATE, HAZARDOUS MATERIALS CARRIED OR SHIPPED should only be completed if applicable to you, NUMBER OF VEHICLES THAT CAN BE OPERATED IN THE U.S. enter current vehicle, equipment & driver information, If hauling only in MN you are an IntraState carrier & within 100-Mile Radius & IS YOUR US DOT NUMBER REGISTRATION REVOKED – click YES OR NO.
Registration forms can also be downloaded or you can request to have a pre-printed form mailed to you by calling FMCSA Support Services at 1-800-832-5660 (Select option 1 (US DOT number questions), then press zero to speak to an agent).
Filing Deadlines - Each motor carrier or intermodal equipment provider must file the appropriate form at the following times: Before it begins operations; and Every 24 months according to the following schedule:

If the next-to-last digit of its USDOT Number is odd, the motor carrier or intermodal equipment provider shall file its update in every odd-numbered calendar year. If the next-to-last digit of the USDOT Number is even, the motor carrier or intermodal equipment provider shall file its update in every even-numbered calendar year.
http://www.irs.gov - Form is required to be filed annually & tax period begins on 07/01-06/30 of the next year. You can print the forms & send with payment to the IRS OR e-file by visiting www.irs.gov/efile - 2290 Tax Returns are due by August 31st of each year (see 2290 instructions for due date confirmation). You may also visit https://app.i2290.com/sign-in to complete your 2290.
This form must be completed & submitted to MNDOT once & an update is required if there are any changes made to your company i.e. switching from ITO to an MTO. Form can be found @ http://www.dot.state.mn.us/const/labor/forms.html Under Vendor Forms & click on Trucking Entity Vendor Form (PDF).
Form must be completed on file with the DVS along with a copy of your current medical card.
